ROFC Premier Registration Forms

We look forward to having your player tryout for ROFC Premier this weekend! 

- In the event that your player is offered a spot on a team, here are the forms that we need so we can properly register your player with the state.

- All of these forms need to be turned in this weekend!  There will be a tent solely for paperwork turn in at the tryouts.  Your player's spot on the team is not confirmed until all paperwork is received by ROFC.

- Please look at the list below and determine which league your player will be in as there are different forms for MYSL , MSPSP and Director's Academy teams - however many team's designation will be determined at tryouts so verify with the coach.

- Most of these forms can be filled out online and printed for your convenience, see the links below for each league's paperwork.  We will also have these forms at tryouts to fill out onsite.

- All pictures must be approximately 1.5" x 1.5" (large 5" x 7" and 8" x 10" will not be accepted).   Also please write your player's name on the back of the picture.

 
- There will be a free notary public present to notarize the Medical Release Form.  However if you can get them notarized in advance it will save you time at the paperwork tent.

- Please do not staple, glue, or tape anything, please use only paper clips.


 


MYSL:    

Teams: Girls - 2010 Green, 2009 Green, 2009 White, 2008 Green, 2008 White, 2007 White, 2006 White, 2003 White

1) MYSL Player Contract (Click link for form)

2) Medical Release Form  (Click link for form)
- This needs to be notarized, we will provide notary services at the tryouts.
 
3) 1.5" X 1.5" picture of your player (large 5" x 7" and 8" x 10" will not be accepted)
- This needs to be small enough that it can be cut down to fit in the box for the photo on the Player Contract and still show  
           the entire face.
- Write your child's name on the back.

4) If you are new to MYSL, you will need to bring a copy of a proof of age (birth certificate, passport or similar government document)
- This needs to be a photocopy that can be turned in, DO NOT bring an original document that you need back.

- Please do not staple, glue, or tape anything, please use only paper clips.



MSPSP: 

Teams:  Girls - 2005 Green, 2004 Green, 2004 White, 2003 Green, 2002 Green, 2001 Green. 1999 / 2000 Green
Boys - 2005 Green, 2004 Green, 2003 Green, 2002 Green, 2001 Green, 1999 / 2000 Green

Director's Academy

The Director's Academy will not accept player registration forms via physical copies or emailed PDF documents. Instead, only player registration documents will be completed through an online form.

This link to the online forms will be sent to the parents after your player has been offered a spot on a team.  Please be prepared to upload a birth certificate or passport as proof of birth and a color photo when that time comes.

Although we are not collecting paperwork for Director's Academy players at tryouts, you still must stop by the Commitment Tent to accept a spot on a team if offered.
